In Madhya Pradesh, providing primary healthcare services has long posed challenges due to either varying geography or administrative lethargy. Nonetheless, the winds have started to change, as the National Health Mission (NHM), in collaboration with the Government of Madhya Pradesh, has pioneered a transformative teleconsultation program.

Spearheaded by 2009-batch IAS officer Priyanka Das, the Mission Director of National Health Mission, Bhopal, this innovative initiative aims to bridge the healthcare gap in rural and tribal areas, where access to specialised medical personnel is limited.

UNIQUE APPROACH

Madhya Pradesh faced unique challenges in providing healthcare to its rural and tribal communities, exacerbated by the COVID-19 pandemic. The spotlight on the region revealed a rising prevalence of Non-Communicable Diseases (NCDs), necessitating a radical approach to healthcare delivery. Recognizing this need, NHM and MP government initiated the ‘Specialist Teleconsultation’ program to bring specialised healthcare services to the grassroots level.

Setting itself apart from national telemedicine trends, Madhya Pradesh’s initiative takes a doctor-to-doctor approach. With a vast shortlist of specialists stationed in major cities like Delhi, Noida, Lucknow, Kolkata, and Jaipur, these urban hubs become the source of specialist consultations for the state. The 1200+ Primary Health Centres (PHCs) act as the point of contact, staffed by MBBS doctors who lack specialisation but facilitate the teleconsultation process.

“This innovative model has transformed healthcare delivery, with over 9500 specialist teleconsultations conducted daily,” Ms. Priyanka informed Indian Masterminds.

The program, launched in 2021, began in three divisions and later expanded statewide due to its success. Currently, three specialties – gynaecology, paediatrics, and general medicine – are provided at the grassroots level.

ADDRESSING CRITICAL HEALTHCARE NEEDS

The teleconsultation program emerged as a game-changer, particularly in identifying high-risk pregnancies and addressing childhood illnesses. Gynaecological interventions have allowed for the early identification of high-risk mothers, preventing complications.

In rural areas, with a shortage of paediatricians, the program ensures proper diagnosis and treatment for childhood illnesses, which were previously overlooked or mismanaged.

INFRASTRUCTURE AND INTEGRATION OF AI

No stone has been left unturned in bolstering infrastructure at PHCs, providing essential amenities and diagnostic tools.

“Though we are currently not leveraging AI, the program utilises an analytical dashboard to understand health trends and potential deficiencies,” Ms Das informed.
The prospect of incorporating AI for more in-depth analysis is on the horizon, pending the accumulation of sufficient data.

ACCOUNTABILITY AND ACCESSIBILITY

Addressing concerns of potential over-medication or deviation from government protocols, Ms. Das informed that the program has implemented safeguards. She shared that the Doctors can only prescribe medications listed in the state’s essential drug list, ensuring adherence to government guidelines. Any deviations require direct communication, with only one such case reported to date.

SCALABILITY AND POTENTIAL

The officer further said, that “this model is highly scalable and not limited to Madhya Pradesh. States facing topographical or human resource challenges can adopt this approach to revolutionise healthcare delivery.”

The success of the program has set a precedent for leveraging technology to overcome geographical barriers and ensure healthcare access for all.

As the program continues to evolve, it is setting a benchmark for other states to emulate, ultimately paving the way for a more inclusive and accessible healthcare system across the nation.
